As of August 1, 2023, we have contracted with a Medical Records Custodian, Shoreline Records Management, Inc.
Authorized requestors will request their records from Shoreline by accessing their website:
https://www.shorelinerecordsmanagement.com/records-request
Fill out the online Medical Records Release Form.
When Shoreline receives the form back, they will notify the requester of the processing fee.
They charge $25.00 per chart. Once payment is received, they will send a secure link via email to the requestor, who can then access and download the chart, or patients may request a CD mailed to their address for an additional $10.00 fee.
Please note that Shoreline will only release a copy of the full chart.

James W. Alexander MD, FACOG
Dr. James Alexander earned his medical degree at University of Vermont College of Medicine in Burlington, VT, and completed his residency at the University of Rochester, Rochester, NY, one of the most highly regarded OB/GYN programs in the United States. Dr. Alexander is Board Certified in Obstetrics and Gynecology. He has served as a mentor to many local physicians. Dr. Alexander has served as the Interim Chief of Obstetrics and Gynecology at Upstate University Hospital at Community General, Vice Chief, and is the Administrative Director of the Obstetric Service.

Danielle LeFebvre, WHNP-BC
Danielle LeFebvre is a Board Certified Women's Health Nurse Practitioner. She earned her medical degree at Vanderbilt University in Nashville, TN and began her nursing career over 10 years ago. Danielle is dedicated to providing quality individualized care to meet all your needs. Her transition as a new provider at our office has been seamless, she is the perfect addition to our team!

I am a board certified Women's Health Nurse Practitioner and began my career in the nursing profession over 10 years ago. My passion of caring for others extends beyond the workplace as my family continues to grow, outnumbered by pets! We currently have two dogs and a rabbit.
Originally from Bufallo, NY, I am pleased to join Dr. James Alexander at Contemporary Obstetrics and Gynecology. His reputation precedes him and many of his patients have been kind enough to share wonderful memories created over the years. I have enjoyed getting to know our existing patients and warmly welcome all of our new ones.
As you consider your health care needs, I express to you my commitment to providing quality, individualized care. I will work with you to tailor a treatment plan that addresses your needs and most importantly that you leave each appointment with your questions answered. Whether you need an annual exam, are experiencing irregular or heavy menstrual bleeding, have questions about contraception or pregnancy planning, are experiencing pelvic pain, painful intercourse, or menopausal symptoms, myself and our office staff are available to facilitate your appointment.
